I am the parent or legal guardian of the player(s) named on this form, and I give permission for them to take part in MCC Soccer Academy training sessions, games and related activities.
I understand that soccer is a physical, contact sport and carries a risk of injury — including sprains, broken bones, concussion and, in rare cases, serious injury. I confirm that my child is in good health and fit to take part, and that I have told the academy about any medical condition, allergy or injury it should know about.
In an emergency, I authorise MCC Soccer Academy coaches to seek medical treatment for my child if I cannot be reached, and I accept responsibility for the cost of that treatment.
To the fullest extent permitted by law, I release MCC Soccer Academy, its coaches and volunteers, and the Muslim Community Center of Greater San Diego from claims arising from my child's ordinary participation, except where caused by gross negligence or wilful misconduct.
I agree that my child will follow the coaches' instructions and the academy's code of conduct, and that the academy may ask a player to sit out for safety or behaviour.
